1889 Foundation, Inc., with 1889 Jefferson Center for Population Health, United Way of the Laurel Highlands, Community Action Partnership, Beginnings, Inc., Alleghenies United Cerebral Palsy, Greater Johnstown School District and Communities in Schools, plans to organize around a community-wide plan called Vision Together 2025 and use their community’s strong voice to improve the health outcomes of their county!

The Vision Together Health and Wellness Team will address a number of health-related community needs through the 1889 Jefferson Center for Population Health. Some of the ways the Center plans to accomplish their goals is through the implementation of the Pathways Community HUB model to improve care coordination. A Community Advisory Council will guide the HUB to make sure that community voice is central to the planning and implementation of this work and that they are actively meeting their community’s priorities. Community engagement is also a priority for the team, as they seek to engage residents to develop a team of Community Health Workers. The initiative is both transformative and collaborative and will work to empower Cambria residents to lower the risk for chronic disease, improve access to care and establish a collaborative food security coalition.
